General settings of form templates
In step 2 you define how completed forms are labelled in the overview list – in the app and in the web interface. This lets you and your team find any report instantly, without having to open it first.
The title and subtitle can be made up of static text and dynamic placeholders – individually or in combination. Placeholders are automatically replaced with the actual field values when the form is saved.
Example: Inspection report #customer_name# from #form_create_date# becomes e.g. “Inspection report Miller Ltd from 06/07/2026”
A full list of available placeholders can be opened directly in the form designer via the link above the fields.
Title in the form overview: The first line shown in the overview list. If no title is entered, Formtastic uses a default name.
Subtitle in the form overview: The second line in the overview list – ideal for supplementary information such as address, job number or date.
